Club waiver form is a mandatory requisite before participation.
Below are two verions of the Club electronic waiver, for adults and minor.
Please fill these online prior to coming down to your first practice.

18 Years of Age & Above

               * note, there is about a 3% PayPal fee in added to the amount.
                Thus (the $100 is $104 in PayPal, and so on)

Early 2012 Membership (before 15 April 2012)	$100
Regular Membership (after 15 April 2012)	$125 + 3.37% Paypal Fee
